What Can You Find About Your Provider on a COHPC Profile?

If a provider answers “Yes” to a profile question, then additional information associated with that “Yes” answer will be shown in a supplemental box below the question. Below are items that will have supporting documentation that you can view.

Disciplinary Actions

Find out if your provider has ever been disciplined in any state or country. If the provider’s answer is “Yes,” you will see specific year(s) and location(s).

Restrictions & Suspensions

Find out the location and date of any restriction and/or suspension for your provider. This may include any voluntary agreement and/or suspension of your provider’s practice. If the provider’s answer is “Yes,” you will see the specific year(s) and location(s).

DEA Registration/ Surrender

See if your provider has ever had to surrender his or her license to the Drug Enforcement Agency. If the provider’s answer is “Yes,” you will see the year of surrender.

Criminal Convictions

Find out if your provider has ever had a final conviction of a felony or crime of moral turpitude since he or she was issued a license. If the provider’s answer is “Yes,” you will see the specific year(s), location(s), and jurisdiction(s).

Malpractice Carrier Refusal 

Find out if your provider has ever been denied liability insurance coverage or had coverage terminated. If the provider’s answer is “Yes,” you will see the year of the refusal.

How to View Copies of Documents

  1. After searching for your healthcare professional at Verify a Colorado License, click “Detail” next to the healthcare professional for whom you are searching.
  2. At the bottom of the “License Details” screen, you will see a section titled “Online Documents” with a box below it.
  3. Click on “DPO Public Documents” to go to the “Division of Professions and Occupations Public Documents Search Form” page.
  4. To view a copy of a provider’s public document, type in your provider’s information or follow the instructions on the bottom of the page. You will see a listing of all documents associated with your provider.
  5. Click on the “Doc Id” number next to the document you wish to view.
