Colorado DPO Application Preview

All Colorado license, registration and certification applications are all available through our DPO Online Services. Instructions on how to create an account or sign in to an existing account are available on the system:

The tabs below represent example sections and questions that are available in each application. For the purposes of this Application Preview the term "License" is used to represent License, Registration, Certification, Credential, Authority, etc. For the license type specific sections, the questions will vary based on the professions requirements. For specific license type requirements, please view the Licensing Guide available on your professions applications webpage. All items listed in italics are general items that may be included in the application based on the professions requirements.

Throughout each application there are several section skips based on selections earlier in the application. For example, if you select your license method as "Examination" your will skip the sections for "Endorsement". Questions and sections are designed to only capture the information necessary to validate and process depending on your credential type and license method. The tabs below will include all options regardless of selections to provide a full view of the questions included in our Online Applications.

License Application Preview

This section is included on all applications and serves as a welcome page as well as a high level checklist of items that an applicant will need to provide. All items listed in italics are general items that may be included in the application based on the professions requirements or reference a general context of the question.

License Application Welcome Section

Welcome to Online (License Type) Application. Before you begin, please review the important information below:

There may be multiple methods you may use to become licensed. To apply by one of the available methods you will have to have already completed, or have in your possession verification of the below. Please review the (License Type) Licensing Guide for more detailed information on requirements.

(License Type) by (Original, Examination, etc.) Method:

  • Examples here will list the various requirements specific to an Original or Examination license method. These items may include, but are not limited to, some or all of the below:
    • School and Education information including uploading copies of transcripts. Some foreign educated applicants may be required to submit additional uploads for credential verification.
    • Completion and passing of exam(s) information attestations or uploads or confirmed registration for certain exams.
    • Specific association or certification participation/completion attestations or uploads.
    • Providing certain professional/national organization ID numbers if necessary.
    • Liability or other insurance attestations or uploads.
    • Completion of internships, training hours, supervision hours, etc. along with attestations or uploads.
    • For license types that require fingerprint background checks, having scheduled with an approved CBI fingerprinting vendor.
    • For license types that have compacts with other states, understanding and attesting to the requirements of the compacts.

(License Type) by (Endorsement, License Transfer, Credentialing, etc.) Method:

  • Examples here will list the various requirements specific to an Endorsement, License Transfer or Credentialing license method. These items may include, but are not limited to, some or all of the below:
    • Possession of an active current license in the profession that you are applying from another U.S. State or Jurisdiction with the ability to provide verification information and upload a copy of your license. Some professions also require active practice in that state for a certain amount of the preceding year(s).
    • Specific association or certification participation/completion attestations or uploads.
    • Providing certain professional/national organization ID numbers if necessary.
    • Completion and the ability to provide verification of any necessary continuing education or competency in the profession.
    • Passing certain Colorado or national jurisprudence examinations.
    • Liability or other insurance attestations or uploads.
    • For license types that require fingerprint background checks, having scheduled with an approved CBI fingerprinting vendor.
    • For license types that have compacts with other states, understanding and attesting to the requirements of the compacts.

Basically, if you don't have the above, it is best for you to wait until you can secure it. Otherwise you will not be able to qualify for an (License Type) license. We'll ask more specific information later in the application about the above items. Remember, you can stop and cancel this application at any time before submitting. However, if you submit the application, pay the fee and do not provide the information as requested, submit incomplete documentation or do not qualify, your application may be rejected and no refunds or transfers will be given.

Applicant Information Section

  • To begin, we need to know a little more about you. The questions below help us to learn about you and provide some information needed in order to process your application as quickly and efficiently as possible. Applicant Previous Names Section

  • You indicated on the previous page that you have had a legal name change, used an alternate name in the past or have you ever practiced under a different name than you registered your account with. Select the "add" button in the grid below to enter the name(s) used previously:
    • Select the "add" button and enter in previous name information.
  • Please upload all legal name change documentation to assist us in verifying your credentials to process your application:
    • Upload the appropriate document(s).

Applicant Military Information Section

  • Are you an active member of the U.S. Military, National Guard or Military Reserves?
    • Select Yes or No.
  • Are you a Veteran of the U.S. Military
    • Select Yes or No.
  • Are you the spouse of an active military member who has been relocated to Colorado AND currently hold a valid and Active credential to practice your profession in another U.S. state?
    • Select Yes or No.

These sections are included on most applications and the questions below are direct examples of what may be requested. Many of the below questions are similar across all license types with small changes to the wording depending on the profession. The examples below are not exhaustive of each license type. License Type Specific Base Section

The questions below are direct examples of what may be requested in the License Type Base Section. Many of the below questions are similar across all license types with small changes to the wording depending on the profession. The examples below are not exhaustive of each license type. Generally this section will have between 4 - 9 different questions, depending on the license type. For comprehensive information on your profession, please review your Professions Licensing Guide:

  • Have you graduated with a degree, diploma or certificate from an approved (profession) education program, college or university? If you have not graduated yet, you cannot apply.
    • Select Yes or No.
  • Enter the name of the approved (profession) education program, college or university from which you graduated:
    • Enter the name of the school.
  • Select the highest level Degree/Diploma you received:
    • Select from a list of Degrees/Diplomas.
  • Have you registered with the (state or national association/organization)?
    • Select Yes or No.
  • If you answered yes that you have registered with the (state or national association/organization), provide your ID Number:
    • Enter the ID number
  • Prior to practicing as a (license type) in Colorado you must attest to obtaining and maintaining Professional Liability Insurance or be covered by an exemption. By selecting "Yes" below you are attesting that you have obtained and will maintain Professional Liability Insurance or be covered by an exemption.
    • Select Yes or No.
  • To move on to the next part of the application, select your license method in the drop-down box below. (If multiple license methods available)
    • Select from a list of License Methods.

License Type Specific Original/Examination Section

  • Please upload a copy of your (certificate or training from the accredited association or organization for your profession).
    • Upload the appropriate document(s).
  • Have you completed and passed an examination approved by the (appropriate professions associations/organization)?
    • Select Yes or No.
  • Please upload a copy of your official (program education) transcript that includes the degree earned and the completion date. Again, if you have not graduated yet or cannot supply the official transcript, do not apply. Upload only your official transcripts.
    • Upload the appropriate document(s).
  • Upload the (verification of training/competency) form completed by a (supervisor) showing that you have completed (the appropriate length of on job training).
    • Upload the appropriate document(s).

License Type Specific Endorsement/Transfer of Grades/Credentialing Section

  • Do you currently hold an active, equivalent license in Colorado of any other state?
    • Select Yes or No.
  • If you said "yes" to the question regarding holding a (license type) in Colorado or any other state, you must list ALL licenses below:
    • Select the "add" button and enter in all applicable licenses.
  • you must scan and upload verification for all licenses below:
    • Upload the appropriate document(s).
  • Upload the (verification of training/competency) form completed by a (supervisor) showing that you have completed (the appropriate length of on job training).
    • Upload the appropriate document.
  • If you have practiced withing the past (range of years) , please list that information in the grid below:

These sections are included on all initial applications and are designed to gather general screening information. The questions below are verbatim for most applications.

Screening Questions Section

  • Within the past five years, have you engaged in any conduct or exhibited any behaviors that resulted in:
    • An arrest, discipline, sanction or warning?
    • Loss or suspension of any license?
    • Termination or suspension from school or employment?
    • Endangering the safety of others?
    • A breach of fiduciary obligations?
    • A violation of workplace or academic conduct rules?
    • An impairment of you ability to practice in a safe, competent, ethical and professional manner?
    • Abusing or excessively using any habit forming drug, including alcohol, or any illegal or controlled substance resulting in any discipline for misconduct, failure to meet professional responsibilities, or affecting your ability to practice safely and competently?
    • Claiming the illegal use of a substance as a defense, in mitigation, or as an explanation for any conduct that impairs your ability to practice in a safe, competent, ethical, and professional manner?
  • For each "yes" response above you will be required to provide:
    • A description and explanation of the behavior(s) or practice(s).
    • Dates of the event(s).
    • Location(s)/Court(s).
    • Current status(es)/outcome(s).
    • Any accompanying documentation.

Screening Inquiry Questions Section

  • Have you ever had any inquiry, investigation or administrative/judicial proceeding by:
    • A Licensing Authority other than a Colorado State Board or Program?
    • A Government Agency?
    • A Court?
    • An Employer?
    • An Educational Institution?
    • A Professional Organization?
    • In connection with an employment disciplinary or termination procedure?
  • For each "yes" response above you will be required to provide:
    • A description and explanation of the behavior(s) or practice(s).
    • Dates of the event(s).
    • Location(s)/Court(s).
    • Current status(es)/outcome(s).
    • Any accompanying documentation.

Screening Healthcare Questions (Selected Healthcare Professions Only)

  • Have you ever had the below occur:
    • Been refused malpractice insurance, had malpractice insurance canceled or rated at a higher premium due to past claims experience?
    • Had your staff membership or clinical privileges at any hospital or healthcare facility, or your DEA registration been reduced, limited, placed on probation, not renewed, relinquished, denied, revoked or suspended?
  • For each "yes" response above you will be required to provide:
    • A description and explanation of the behavior(s) or practice(s).
    • Dates of the event(s).
    • Location(s)/Court(s).
    • Current status(es)/outcome(s).
    • Any accompanying documentation.

These sections are included on all initial applications and are designed to application attestation information. The questions below are verbatim for most applications.

    • False statements made on my application could result in a violation of the practice act.
