How to Change an Address/Contact Info for Individual Licenses
Individuals may change their address online using DPO Online Services System. There you may update your mailing address, phone number, email address and conduct a variety of business with the Division regarding your license.
How to Change an Address/Contact Info for Business Licenses
Businesses and entities may not change their name or address online. The entity will need to submit a paper application/form for this change to be effective. Please reference the board or program that your business or entity is associated with looking for the specific program's application and forms page. You may find your programs home page by visiting the DPO Home Page and selecting the appropriate program.
How to Change Your Name with the Division
All Individual licensees wishing to change their name with the Division must complete a Name Change Request Form.
A copy of one of the following must accompany all Name Change Request Forms for individuals. DO NOT SEND ORIGINALS:
- Marriage License
- Divorce Decree
- Court Order
How to Update/Provide a Social Security Affidavit
All applicants who do not have a Social Security Number must complete an Affidavit of Social Security. This form is only for licensees who meet the following criteria:
- Not physically in the United States.
- Non-immigrants in the United States on a student visa.
- Non-immigrant P-1 individual athlete in the United States on an authorized stay pursuant to Title 8, Section 214.2(p) of the Code of Federal Regulations and Section 214(a)(2)(B) of the Federal Immigration and Nationality Act.
