Optometrist Applications, Instructions and Forms:
The following applications are available by logging on to the DPO Online Services system. There you can apply for an initial license, Reinstate an Expired license, Renew your license, update your contact information, review the progress of your application and much more. Instructions on how to create an account or sign in to an existing account are available on the system:

Communication from the Division: The Division of Professions and Occupations primary communication is via email. Please be sure to include your current email address on your application to receive important information about your application status and process.
License Expiration Bump Period for New Applicants: All new applicants who are issued a license within 120 days of the upcoming renewal expiration date will be issued a license with the next expiration date.
License Renewal Information: All Optometry licenses expire on March 31 of odd-numbered years and must be renewed to continue practicing. View your expiration date by logging onto your DPO Online Services Account. Please note that renewals open approximately 4-5 weeks prior to the expiration date.
