Q: When does a Governing Board have to register with DORA?
A: A professional review governing board ("governing board") that establishes or uses one or more professional review committees to review the practice of persons licensed under Article 240 or licensed under Article 255 and granted authority as advanced practiced nurses or certified midwives shall register with the Division of Professions and Occupations (DPO) within 30 days after approving the written bylaws, policies, or procedures for the professional review committee.
Q: How do I register a Governing Board with DORA?
A: First, search the List of Registered Governing Boards to ensure that your governing board isn't already registered with DORA. If your governing board is not registered, proceed with registering.
To register, access the Professional Review Governing Board Registration System, create an account and follow the prompts to enter the information about your governing board. We recommend that different names and email addresses are entered for the "Primary Contact" and "Alternate Contact". The person who registers the governing board is considered the "Responsible Individual". The "Responsible Individual" will be able to make changes/updates and submit an annual report.

Q: How do I update our contact information?
A: If you know the Logon ID and Password, log in to the Professional Review Governing Board System. Once logged in the system, select the tab "Update", make all changes and select the tab "Save".

A: If you have forgotten your password, use the "Password Finder" feature on the Professional Review Governing Board System Password Finder Page. An email will be sent to the email address on record with information on how to reset your password.
- Information about Annual Reporting
Q: When do I submit my annual report?
A: Online reporting opens on January 1 and closes at midnight on March 1. Your report must be submitted in the Professional Review Governing Board System before midnight on March 1 for reporting data from the previous calendar year.
Q: What information will I need in order to submit my Annual Report?
A: When you submit your Annual Report you will be asked to input the total number for each of the categories below (the reporting period is January 1 through December 31):
Professional review activities or investigations* in which no final action adversely** affecting the individual was taken/recommended:
- Number of investigations completed during the year
- Number of investigations that resulted in no action
- Number of investigations that resulted in written involuntary requirements for improvement
- Number of written agreements for improvement between the subject of the investigation
*An investigation occurs when the authorized entity or its professional review committee notifies the subject of the investigation in writing that an investigation has commenced.
**Adversely affecting means reducing, restricting, suspending, revoking, or denying clinical privileges or membership in a health care entity (45 CFR 60.3) except that it does not include a precautionary suspension or any professional review action for a period of thirty days or less.
Medical Board Review Actions - for persons licensed under article 36, report the number of professional review actions that meet each criteria:
- Adverse Actions - number of review actions adversely affecting the individual
- Surrender During Investigation - number of review actions in which an individual surrendered clinical privileges, membership, or affiliation while the individual was under investigation
- Surrender Without Investigation - number of review actions in which an individual surrendered clinical privileges, membership, or affiliation in return for an investigation not being conducted
- Recommendations Following Hearing - number of review actions in which the professional review committee made recommendations regarding the individual following a hearing
Nursing Board Review Actions
- Adverse Actions - number of review actions adversely affecting the individual
- Surrender During Investigation - number of review actions in which an individual surrendered clinical privileges, membership, or affiliation while the individual was under investigation
- Surrender Without Investigation - number of review actions in which an individual surrendered clinical privileges, membership, or affiliation in return for an investigation not being conducted
- Recommendations Following Hearing - number of review actions in which the professional review committee made recommendations regarding the individual following a hearing
Q: Where can I find the aggregate information from the Professional Review Governing Boards registered with DORA?
A: After the reporting period closes, a report containing aggregate data, and without individually identifiable information concerning a governing board, will be available on the Professional Review of Health Care Providers website under Annual Reporting on the Program homepage.
Q: How do I submit an Annual Report?
A: An annual report must be submitted online, it cannot be mailed. Log in to the Professional Review Governing Board System using the User ID and Password created when the governing board was registered with DPO. The person who registered the governing board is considered the "Responsible Individual" and can make changes/updates and submit annual reports.

Once logged in to the record for your governing board, you will see a tab at the bottom of your page "Submit Report". Select "Submit Report" and follow the prompts to submit your Annual Report.
